Office Hours and Appointments

We see patients by appointment only. However, we try to be flexible for urgent patient needs. Our office hours are 8:30 a.m. to 5:00 p.m., Monday through Friday. We schedule appointments between 8:30 a.m. and 4:00 p.m. We make every effort to stay on time, realizing that your time is valuable too. We do not address walk-in's at this time. Please note that if you are more than 20 minutes late for your appointment you may be asked to reschedule.

New Patients

A "new patient" packet of information was sent to the address provide to our office by your referring physician. Please complete the enclosed paperwork prior to your visit and bring it with you to your scheduled appointment. We have enclosed a Registration Sheet, Consent and Authorization Form and a Comprehensive Patient Questionnaire for you to complete. Please carefully complete all questions to the best of your knowledge. Failure to do so may require us to reschedule your appointment. Please do not mail your paperwork back to our office!

If you have not received this packet or have misplaced it, a copy may be printed from this website. Please see the "Forms" tab for your convenience.

Cancellation Policy

Office Appointments:

If you must cancel or reschedule your appointment for any reason, please call as early as possible; preferably at least 48 hours in advance. Missed appointments are subject to a prepayment charge prior to rescheduling and cannot be filed on insurance. Multiple missed appointments may result in the practice no longer being able to care for the patient.
